Fill in the form below…  Your ad will be generated from the text you provide.  To create more than one ad in a single submission, please be sure to clearly delineate what text belongs to each ad and its Donor / Sponsor / Patron level.  You may submit each ad individually by selecting only 1 “ticket” so that you are billed for just one at a time.

All camera ready art must be emailed to karen.g.deutsch@gmail.com.  Check the box photos to be sent by email when sending photos so that I will know that your ad includes more than the text you are including here.
